Participants

Please note that registration for this program is full. Although you are welcome to submit an application, if you are accepted you will be placed on a waiting list.
To be considered for review, we must receive all of the following:
1.	A completed application form. (All applicants.) Apply online. Please use this option if possible. Download a printable RTF version to submit by fax or post. This document can be opened and printed using Microsoft Word. Instructions on where to mail or fax your application are located on the form itself and in the program fact sheet.
2.	A recent photo. (First-time applicants only.)
3.	An application fee of $25.00 USD. (First-time applicants only.)
4.	1 letter of recommendation from the following people (First-time applicants only.): your meditation instructor

Recommenders

There are several ways to submit a recommendation on behalf of a student. Be sure to review the recommendation guidelines before meeting with the applicant to discuss the recommendation. The guidelines are located with the recommendation forms below. The following options for submitting your recommendation are listed in order of preference:
1)	Submit the recommendation online. Either follow the link sent to you by the student (easiest) or click here to fill out your recommendation online. Please note that you will be asked for your shambhala.org member ID and password when you follow this link. If you have forgotten or don't yet have a password, click here. Please use this option if you can. It is most expedient and guarantees that your recommendation is immediately available to the reviewers.
2)	Submit your recommendation by fax. Click here to download a Word (RTF) version of the recommendation form.
3)	Send us your recommendation by post. Please do this only if the other options do not work for you. Click here to download a Word (RTF) version of the recommendation form .
